Today, December 1st, is World AIDS Day, a day that is all about raising awareness, countering prejudice, and helping stop the spread of HIV/AIDS.  MobileActive.org has put together some of the mobile projects and organizations we've covered recently that are innovating with mobile tech in the fight against HIV and AIDS.

RedChatZone: HIV Counseling via Mobile Instant Messaging Chat

This project is an innovative mobile-based platform for youth and young adults to learn more about HIV and to get support by offering them the ability to communicate anonymously and privately via MXit with a trained counselor.

Over 5 million people in South Africa are living with HIV and the country has the highest burden of TB-HIV co-infection. TB treatment completion and cure rates fall below 50% in almost half of the districts. Project Masiluleke, Zulu for to give wise counsel or lend a helping hand, stepped up to the challenge and is using mobiles to provide end-to-end care through awareness, testing, and ensuring treatment compliance.

Monopolizing on the 90% of South Africans who own mobile phones, the iTEACH team -- with collaborators from Pop!Tech, The Praekelt Foundation, and Frog Design -- brought HIV awareness to the masses and improved treatment compliance through the use of text messaging. Social stigma towards those carrying the disease results in low rates of HIV testing, and an overburdened public health system discourages people from utilizing available services at local clinics.

iTEACH identified four links in a chain of care essential for HIV and TB treatment success: [1] Effective awareness, [2] Scale-up of early HIV testing, [3] Early anti-retro viral treatment (ART) initiation, and [4] Support for ART and TB treatment adherence. In October 2007, Project ‘Masiluleke’ (Zulu for ‘to give wise counsel’ or ‘lend a helping hand’) or ‘Project M’, was launched to address these challenges with a chain of interventions designed for replication and scale. Project M is a collaborative effort, lead by the iTEACH NGO, along with a diverse set of partners (The Praekelt Foundation, Frog Design, Pop!Tech, MTN and Vodacom, LifeLine Southern Africa, Ghetto Ruff Records and the National Geographic Society).

Brief description of the project: 

Briefly, each of the three components of Project M addressed a specific barrier for people seeking care and for health care workers managing care.

1. TxtAlert: Reminders for HIV clinic appointments were sent to patients’ mobile phones as text messages. Showing up to your HIV clinic appointment largely correlates with anti-retro viral treatment (ART) adherence. In June 2007, TxtAlert, an SMS-based automated ART clinic appointment reminder system was piloted in partnership with Right to Care at the Themba Lethu Clinic, the largest privately funded ART treatment site in South Africa. TxtAlert is a web service that is linked to the electronic medical record system.

Clinic appointment reminders are sent to patients who are receiving ARVs, both two weeks and one day before their scheduled appointments. Text messages are also sent one day after the appointment to either thank the patient for coming or to alert them of a missed appointment and encourage rescheduling.

2. SocialTxt: A mass mobile messaging campaign was launched to raise HIV awareness, and directed people to the National AIDS Helpline. In October 2007, a mass text message HIV awareness campaign was launched with the so-called SocialTxt technology developed by the Praekelt Foundation.

One million text messages were sent everyday to mobile phone users throughout South Africa that contained key information about HIV and TB and directed mobile phone users to a free National AIDS Helpline.

The campaign resulted in an impressive 300% increase in calls to the helpline, from 1000 calls a day to 3000-4000 calls a day. Since the project launch, over 685 million PCM messages were sent to the helpline. Upon receiving the PCM message, staff at the helpline, who are HIV patients themselves, offer counseling and direct callers to HIV and TB testing centers.

3. HIV Self-Test Kit: Currently in development is a home-based HIV test kit that has the option of calling the National AIDS Hotline for guidance and assistance. The hope is that this kit will enable South Africans to perform an HIV test, in the privacy of their own homes, with the option of calling a counselor at the National AIDS Helpline via cell phone.

Target audience: 

Project M is being piloted in KwaZulu-Natal (KZN), which is the South African province with the highest HIV burden and poorest TB treatment outcomes. The project will be managed from Edendale Hospital, which is located in one of the hardest hit districts in KZN (Umgungundlovu) where 60% of pregnant women test HIV+ and 200 new cases of TB are registered every month. Edendale serves an exclusively Zulu population of 1 million persons, where unemployment is estimated at 60% and most are living in abject poverty.

Edendale was selected specifically because it is replete with challenges faced by large government hospitals at the epicenter of the HIV and TB crisis. Success in this setting would suggest that replication and scale-up in similar underserved communities is feasible.

What worked well? : 

1. TxtAlert: HIV/AIDS Clinic Apppointment Reminders

  • 80-90% opt-in rate was observed at the clinics.
  • Patients started to rely on TxtAlert reminders so much so they were less likely to switch their mobile phone number.
  • Patients started to interact with TxtAlert system via "Please Call Me" messages to reschedule their appointments in advance.
  • Patients currently on ARVs at the pilot site assisted in encouraging patients to opt-in for the SMS alerts.

2. SocialTxt: Mass mobile campaign to raise HIV awareness and direct people to the National AIDS Helpline

  • iTEACH offered relevant clinical and cultural expertise for the content of the SocialTxt SMS messages.
  • The project increased the number of calls by 300% to the National AIDS Helpline.
  • It leveraged the massive uptake of "Please Call Me" messages in South Africa for a large public health messaging campaign at no cost to people.

3. HIV Self-Test Kit, with option of calling via mobile phone to the Helpline

  • Focus group discussions revealed people prefer telephone interactions to seek counseling and HIV testing support, as opposed to in-person consultations.
  • Mobile phones address the issue of perceived stigmatization at clinics, which deters people from getting tested.

 

What did not work? What were the challenges?: 

1. TxtAlert: HIV/AIDS Clinic Apppointment Reminders

  • The process of collecting and registering numbers of patients was tedious, though outcomes show it was well worth it. This process took about 3 months.
  • There is a need to confirm at each appointment that the patient's number is the same as that listed on their system. Many people switch between 2-3 SIM cards.

2. SocialTxt: Mass mobile campaign to raise HIV awareness and direct people to the National AIDS Helpline

  • There are challenges in securing mobile operator support, though this changed once a profitable business case was demonstrated.
  • Counselers at the helpline need more training to deal with the increased volume in calls and to ensure the quality of service is not compromised.
  • The service needs to have increased coverage during evenings and weekend.
  • Counselers need better knowledge of local resources to make effective referrals.
  • A toll-free line is needed so mobile phone users can call without charge. The Helpline is toll-free only if calls are made via landlines.

iTEACH stands for Integration of TB in Education & Care for HIV/AIDS, and is a registered NGO, committed to improving HIV and TB care and treatment for persons living in poverty. The iTEACH team is comprised of 20 local South Africans, and works in close partnership with the Department of Health and leaders in the community to pilot innovative interventions that have measurable outcomes and can be replicated and scaled-up in similar high-burden resource limited settings. For Project M, iTEACH provides clinical and cultural expertise and is responsible for content of the SocialTxt SMS messages as well as field-testing and piloting of the HIV self-test kit and development of the cell phone counseling service.

Mobile Money For Health: A Two-Part MobileActive.org Series

Mobile phones are being tried and tested in myriad ways in health care. They are used for data collection and disease surveillance, for ensuring treatment compliance, for managing health information systems and point-of-care support, for health promotion and disease prevention, and for delivering emergency medical services. Clearly, m-health, as this growing field is dubbed, is here to stay.

At the same time, achieving scale and sustainability in most m-health projects has been a challenge. One of the key aspects of beginning to think about ways to integrate m-health into health systems in a sustainable way is to establish financial systems to pay for health services and to ensure financial accountability within programs.

Abstract: 

In November 2009, the USAID | DELIVER PROJECT office in Zambia, with technical assistance from the home office, conducted the End-Use Verification activity to assess the performance of the logistics management and supply chain systems for selected essential drugs, malaria and family planning commodities.

The survey was used in conjunction with the ongoing Essential Drugs Logistics System Pilot in Zambia, and overall objective was to assess how the logistics systems managed selected commodities at public health institutions, as well as provide a snapshot of how malaria was being diagnosed and treated at lower level facilities. This report presents the findings of the assessment as well as the short- and long-term recommendations to successfully implement the End-Use process, as well as the use of EpiSurveyor for regular data collection purposes.

Abstract: 

In the context of the rapid growth of mobile phone penetration in developing countries, mobile telephony is currently considered to be particularly important for development. Yet, until recently, very little systematic evidence was available that shed light on the developmental impacts of mobile telecommunication. The Information and Communication Technology for Development (ICT4D) program of the International Development Research Centre (IDRC), Canada, has played a critical role in filling some of the research gaps through its partnerships with several key actors in this area.

The objective of this paper is to evaluate the case of mobile phones as a tool in solving development problems drawing from the evidence of IDRC supported projects. IDRC has supported around 20 projects that cut across several themes such as livelihoods, poverty reduction, health, education, the environment and disasters. The projects will be analyzed by theme in order to provide a thematic overview as well as a comparative analysis of the development role of mobile phones. In exploring the evidence from completed projects as well as the foci of new projects, the paper summarizes and critically assesses the key findings and suggests possible avenues for future research.

Abstract: 

Growth in mobile phone penetration has created new opportunities to reach and improve care to underserved, at-risk populations including those with tuberculosis (TB) or HIV/AIDS.

This paper summarizes a proof-of-concept pilot designed to provide remote Mobile Direct Observation of Treatment (MDOT) for TB patients. The MDOT model combines Clinic with Community DOT through the use of mobile phone video capture and transmission, alleviating the travel burden for patients and health professionals.

Three healthcare professionals along with 13 patients and their treatment supporters were recruited from the Mbagathi District Hospital in Nairobi, Kenya. Treatment supporters were asked to take daily videos of the patient swallowing their medications. Patients submitted the videos for review by the health professionals and were asked to view motivational and educational TB text (SMS) and video health messages. Surveys were conducted at intake, 15 days, and 30 days. Data were collected in 2008 and analyzed in 2009

All three health professionals and 11 patients completed the trial. All agreed that MDOT was a viable option, and eight patients preferred MDOT to clinic DOT or DOT through visiting Community Health Workers.

MDOT is technically feasible. Both patients and health professionals appear empowered by the ability to communicate with each other and appear receptive to remote MDOT and health messaging over mobile. Further research should be conducted to evaluate whether MDOT (1) improves medication adherence, (2) is cost effective, and (3) can be used to improve treatment compliance for other diseases such as AIDS.

Decision making support for nurses and health workers, even when connectivity is poor or low, is possible with Sana Mobile, an Android-based mobile health application. Formerly known as Moca Mobile, the Sana technology facilitates remote consultations between health care specialists and community health workers in remote areas.

Sana Mobile started at MIT's NextLab, where developers, faculty and students collaborate to tackle a problem using mobile technology. The Sana technology was developed by Sidhant Jena, Sana team lead and Harvard Business School student and Russell Ryan, lead engineer and MIT student.

When general practitioners lack the expertise to diagnose a case, they refer patients to specialists, who may not be easily accessible. The Sana technology addresses the lack of accessibility to specialty care in places, where specialist doctors and tertiary care centers are sparse.

One of the largest problems facing the developing world is a lack of trained physicians. While there is not a shortage of untrained or semi-trained workforce, many health workers in many developing nations are not able to dispense adequate care due to a lack of expertise.

The Sana technology seeks to address the lack of accessibility to specialty care in places, where specialist doctors and tertiary care centers are sparse.

Brief description of the project: 

Oral cancer is one of the most common cancers in India. It is related to the cultural practice of chewing paan masala and gutka, forms of chewing tobacco. It is estimated that about 40% of men and 15% of women chew tobacco on a regular basis. Detecting risk of oral cancer earlier is important for affordable treatment as late stage tumors can be very expensive to treat. To survey the risk of oral cancer in their community, Mazumdar Shaw Cancer Center and the Narayana Hrudayalaya Health to ran a pilot test in and around Bangalore over six months. One pilot was run in Belgaum, Raichur, a rural town about 12 hours away from the city of Bangalore. The other pilot was run in Bangalore. Equipped with the Sana application, ASHAs (community health workers) went door to door in and around Bangalore, asking questions and taking pictures of oral lesions, which were evaluated by oral surgeons. In six months, they were able to screen about 400 high-risk cancer patients and detected numerous lesions using Sana. Twenty ASHAs were trained in using the Sana software in Belgaum.

Target audience: 

For this pilot, risk of oral cancer was assessed for men and women chewing tobacco. But the Sana application is also being used to do a large scale risk assessment for cardiovascular disease, to screen maternal complications (e.g. preclampsia) and assess nutritional status of children.

  • Sana invested in building local developer capacity in India. Local developers cost less than having an international technology consultant to conduct training.
  • Lots of enthusiasm from leaders at the Narayana Hrudayalaya hospital
  • Funding for Sana developers at MIT to travel to India to test and train users came from external support, not from program budget of the clients.
What did not work? What were the challenges?: 
  • Semi- to low-literacy amongst community health workers can be a challenge if an application is available in limited languages. CHWs in Belgaum did not speak Kannada (the local script) or English. Developers adapted Sana to replace text in the medical procedures with voice prompts and pictorial labels
  • Health workers/nurses were not familiar with the touchscreen and needed some additional training and practice in order to understand how to use it (i.e. using fingerpad or fingernail to enter data)
  • Cost of phone was a major limiting factor
  • Some resistance faced from physicians, who were not quick to adopt a technology-based work flow management tool
  • The Sana Team is a volunteer based organization, and it is difficult for the team of developers to contribute time resources, but they consistently offer feedback to Sana users on their google group.

Sana Mobile

Sana, is a student organization based at the Massachusetts Institute of Technology (NextLab, Center for Transpotation and Logistics, Engineering Systems Division) that offers an end-to-end system that seamlessly connects health workers to medical professionals. In addition, the group assists its partner organization with implementation through collaborative innovation. The team consists of volunteers with diverse background – medicine, computer science, clinical informatics, health policy, social sciences, and business and operations management. It strongly believes that there is a need to bring together experts from various disciplines in order to design an operating system around the delivery of care that is facilitated by mobile technology. The technology is crucial, but not the sole component of the solution. It is the centerpiece of a learning system that is designed not only to improve clinical outcomes, but the health delivery process itself.

The Health Impact Fund (HIF) is a proposal designed to incentivize pharmaceutical innovation by rewarding the development of new medicines in proportion to their health impact. The health care sector is plagued by wastage and poor governance, especially in developing countries. The MHIF system is a mobile payment and data collection system designed to enhance transparency and accountability, whilst improving the quality of data available for impact assessment. It also provides a platform for reporting misuse and abuse. The system is designed to mitigate some of the deployment challenges of the HIF and to act as a springboard for further expansion into broader mobile health (MHealth) solutions.

Abstract: 

Two challenges to successful antiretroviral therapy (ART) scale-up in resource-limited settings (RLS) are human resource and healthcare infrastructure limitations. 

We read with interest the modeling study by Bärnighausen et al. which describes the complexities of ensuring adequate human resources to treat HIV/AIDS (HRHA). The authors suggest that factors needed to achieve universal ART coverage include “changes in the nature or organization of care,” training health workers with skills specific to the developing world to reduce emigration, and developing systems that decrease the number of traditional HRHA required to treat a fixed number of patients.

The Rakai Health Sciences Program (RHSP) PEPFAR-funded ART program has been actively pursuing innovative HIV care strategies that directly address these important points. In 2006, we piloted a novel program utilizing peer health workers (PHW) and mobile phones to monitor patients in a rural ART program in Rakai, Uganda.

Abstract: 

This paper presents a case study of the efficiency gains resulting from the introduction of electronic technologies to monitor and support adherence to highly active antiretroviral therapy (HAART) in Guguletu, South Africa. It suggests that the rollout of HAART to such resource-poor communities can be assisted significantly by the introduction of modified cellphones (to provide home based support to people on HAART and improve the management of adherence data) and simple barcoding and scanning equipment (to manage drug supplies). The cellphones have improved the management of information, and simplified the working lives of therapeutic counsellors, thereby enabling them to spend less time on administration and to devote a constant amount of time per
patient even though their case loads have risen threefold. It has helped integrate the local-level
primary health service provision of HAART with the kind of centralised data capture and analysis
that could potentially support a national HAART rollout.

This White Paper, written by a team of researchers at the Center for Global Health and Economic Development at the Earth Institute, Columbia University, examines and synthesizes the existing mHealth literature to assess the current state of mHealth knowledge, evaluate the impact of mHealth implementations in LMICs and to examine programming, policy and research-related barriers to and gaps in mHealth scale and sustainability.

The paper is divided into two main sections. The first section reviews and summarizes the peer-reviewed literature on mHealth initiatives (focus on LMICS) to highlight trends and challenges. The second section examines the existing mHealth policy environment, barriers and gaps, and key drivers needed for an enabling policy environment.

The major thematic areas include:

  • Treatment Compliance
  • Data Collection and Disease Surveillance
  • Health Information Systems and Point of Care Support
  • Health Promotion and Disease Prevention
  • Emergency Medical Response

The review identified significant gaps in mHealth knowledge stemming from the limited scale and scope of mHealth implementation and evaluation, a policy environment that does not link health objectives and related metrics to available mHealth tools and systems, and little investment in cost-benefit studies to assess mHealth value and health outcomes research to assess success factors and weed out poor investments.

Abstract: 

Over four and a half billion people live in the developing world and require access to services in the financial, agricultural, business, government and healthcare sectors. Due to constraints of the existing infrastructure (power, communications, etc), it is often difficult to deliver these services to remote areas in a timely and efficient manner.

The CAM framework has found success as a flexible platform for quickly developing and deploying high-impact applications for these environments. Many of the applications built with CAM have relied on a model where a field worker with a mobile phone regularly returns from a disconnected environment to one with connectivity. In this connected state, the phone and a centralized server can exchange information and get the collected data backed up on reliable media.

We propose extending CAM’s networking model to enable continual operation in disconnected environments. Using a set of heterogeneous paths made available through social and geographic relationships naturally present among workers, we describe a system for asynchronously routing data in a best-effort manner.

Abstract: 

Results of proof-of-concept testing for an mHealth solution for reproductive health in Uttar Pradesh, India. The text messaging tool based on FrontlineSMS, is a family planning service that relies on the Standard Day Method system of birth control. This method of birth control depends on the woman's fertility cycles to avoid pregnancy. Women can text the date of their menses and user receives their fertility status. Additionally, women can receive information about other family planning options, and support. Presented are observations on the appropriatenss of the technology from focus group discussions. The authors share the input given by target users in the product and service design. Women tested the solution to give insight on feasibility and design.

This paper provides an overview of the implications of this trend for the delivery of healthcare services and population health. In addition to addressing how mobile phones are changing the way health professionals communicate with their patients, a summary is provided of current and projected technologic capabilities of mobile phones that have the potential to render them an increasingly indispensable personal health device. Finally, the health risks of mobile phone use are addressed, as are several unresolved technical and policy-related issues unique to mobile phones. Because these issues may influence how well and how quickly mobile phones are integrated into health care, and how well they serve the needs of the entire population, they deserve the attention of both the healthcare and public health community.

Ths manual give an overview for how to implement and use RapidSMS in a mobile data collection project. RapidSMS is a SMS framework for data collection, group coordination, and complex SMS workflows.  The tutorial outlines when and when not to use RapidSMS, guides the user through project steps and milestones, outlines factors for a successful implementation, and provides worksheets for project planning. Example training materials are included.

Abstract: 

The aim of this paper is to encourage reflection and discussion around the
potential of mHealth in developing countries and to consider how early experiences
can inform the way forward. Toward this aim, I synthesize many reviews and
presentations from the eight years I have been studying the evolution of mobile
phones and health in developing countries. I include observations and discussions
that are now shaping the creation of mHealth as a field, to highlight the ingredients
we need to move from a series of pilot projects and isolated business opportunities
to a full-scale maximization of health-related benefits.

I begin by reviewing the strategic priorities within global health, where mobile
telephony can have the greatest impact, along with organic health-related uses of
mobile phones, and examples of formal mHealth interventions. I then demonstrate
the potential for mobile phones to become an extension and an integral
component of eHealth, describing how information and communication technology
(ICT) can be used in health care, as well as mHealth, as a subset of mServices:
using mobile devices to deliver services such as banking and health. I also show
how trends and interests are converging among key stakeholders within the
mHealth ecosystem, thus forming a foundation on which we can scale up and sustain
more and better mHealth activities. Finally, I present some tactical guidance
for a way forward that will further the objectives of both public health and business,
particularly in outreach efforts to emerging markets, the bottom of the pyramid,
and the next billion mobile phone subscribers.

Abstract: 

Baseline under five child mortality in Sauri, Kenya as of 2005 was estimated to be 148 deaths per 1000 live births. By 2008, the rate had dropped to 81 deaths per 1000 live births due to Millennium Village Project (MVP) interventions. A review of child deaths revealed that among other causes, such as malaria, acute febrile illnesses, diarrheal illnesses and HIV, malnutrition contributed to more than 50% of all child deaths. Community health workers (CHWs) led several interventions, namely community-based management of acute malnutrition, home-based testing for malaria and diarrheal illnesses and immediate dispersal of appropriate treatments.

To support these interventions, MVP ran a pilot project where CHWs were equipped with mobile phones to use SMS text messages to register patients and send in their data with the goal of improving child health and empowering community health workers. This report seeks to detail the methods used, illustrate early results and initial findings of the ChildCount mHealth platform that CHWs have now been using since early July of 2009.

Abstract: 

Presented here is an overview of the operational needs of NGOs and CBOs and the role information systems can play to increase their accountability and efficiency. Information systems need to fit the diverse operational needs of NGOs and CBOs, which include coordinating activities, training and monitoring staff, documenting results, accounting, reporting, decision making and learning, acquiring external information and encouraging community participation.

Unfortunately, there are gaps in information systems that impede the ability of NGOs to embrace ICT. To name a few, the lack of open, accessible, cross-platform mobile development tools, limited opportunities and resources provided to local small software companies to engage with NGOs, and the lack of long distance networking technologies to reach remote locations.

Two examples of technologies that were applied successfully are shared: Self Help MIS, an application to monitor activities of small microfinance organizations and credit groups and DigitalICS, an application for data collection used by agricultural cooperatives.  The author stresses that ICTs should be viewed as a tool to allow local change agents to be more effective and accountable and shows how computing is able to support local organizations by inspiring innovation, implementation and dissemination of projects, and measuring impact.


Praekelt Foundation: Young Africa Live, SocialTXT, and TXTalert

The Praekelt Foundation was founded in 2007 as the nonprofit/NGO offshoot of Praekelt Consulting.  The NGO now runs three programs that work to better the lives of people living in poverty in South Africa. Each of those programs (Young Africa Live, SocialTXT, and TXTalert) use mobiles to achieve that goal.

TXTalert: The goal is to use SMS reminders to increase kept appointment rates at clinics, encourage regular medication for chronic illnesses, and allow patients a free way to contact clinics if they have a problem.

Young Africa Live is a mobile portal where users can access information about HIV/AIDS while also reading entertainment-orientated blog posts.

SocialTXT takes advantage of the unused space in "Please Call Me Messages" to post informative social messages, such as the contact number for the National AIDS Helpline.

TXTalert uses SMS reminders to encourage patients with chronic illnesses to take their medication and follow-up with their clinic appointments. 

Target audience: 

The target audience for all three programs are people living in poverty in South Africa. Young people are a particular target audience of Praekelt's programs.

What worked well? : 

Young Africa Live: The site had rapid pickup among users, and exceeded the expected number of users. The Praekelt Foundation was able to get many resources from NGOs to populate the site with static content, and the bloggers have been well-received by readers.

SocialTXT: The program had a large effect on the number of users calling the National AIDS Helpline, and they were able to incorporate in regional languages in order to make the project more inclusive.

TXTalert: The appointment reminders dropped missed appointment rates at a Johannesburg hospital from 30% to 4%. 

What did not work? What were the challenges?: 

Young Africa Live: The portal is only accessible to users who use Vodacom as a service provider. Thus not all mobile users in South Africa can access the information. Also, the site's rapid popularity created a need for more content.

TXTalert: The system currently only runs in Johannesburg because it is dependent on clinics and hospitals having electronic patient databases, which many rural clinics do not have.
